Shown below is the multi-jurisdiction sales and use tax certificate form. Most states accept this form for tax exemption purposes. Please review this form for the tax requirements in your state. In the event your home state does not accept this form, we have provided a link shown below which indicates the appropriate tax exemption form for your state. Please print and send this form with your credit application.

Credit approval normally takes 3-5 business days depending on the level of cooperation from your references. If time is of the essence, we can ship your opening order using alternative methods such as wire transfer or credit card.
